Just to be clear, there is absolutely no reason the consoles can't use the exact same format for their save files that the PC uses. As long as the different versions of the game are storing exactly the same information as each other, there is no reason why you can't make them interchangeable. I don't know if you actually can though.
They could have prevented this on purpose, or there may be variations in the information that is stored. Assuming you can do this with the vanilla game, not all mods would break this either. As long as the mod doesn't use any scripts, or add or remove anything, it shouldn't show up in the save (as in, modifications to existing assets, such as replacing textures, shouldn't show up in the save and therefore wouldn't break compatability). If you want to try it, I'd load a save from the PS version first.

There may be alterations that need to be made to the file when moving between systems (such as the file extension), and this will be easier to see and do from the PC side. In theory yes, but in practise there appear to be all sorts of problems for reasons that are unclear. I know its possible to transfer Xbox saves for instance but getting them to perform bug-free on a PC is another matter.
